  Complete the First Report of Injury or Illness form and fax to Amy Jackson at (606) 596-1097 to file a claim.

• KEMI Health Care Provider Card

  A copy of the KEMI Health Care Provider Card will need to be provided to the provider in which the injured employee seeks treatment.

• KEMI Prescription Card

  A copy of the KEMI Prescription Card will need to be provided to the pharmacy in the event the injured employee is prescribed medications.
